Coverage
includes employee, spouse, and any dependents up to age 26.

If dental
insurance is elected, employees are responsible for a
minimal premium contribution.
Vision Insurance: VSP
·
Covers
in-network vision exams with a $20.00 co- pay
·
Covers
basic hardware, including contact lenses with a $20.00
co-pay.
Flexible Spending Account(FSA)
·
Allows
employees to set aside a specific pre-tax dollar amount for
unreimbursed medical, dental or dependent care expenses.
Life Insurance: Guardian Insurance Co.

Employees
are eligible for a flat $15,000 basic life and AD&D benefit
·
In
addition, employees have the option to purchase additional
life insurance on themselves (up to 150,000), spouse and/or
children.
401 (k) Retirement Plan :AUL

Eligibility
for participation in the 401 (k) plan requires completion of
6 months of service with CDM, effective the first day of the
following quarter.
